Question:

I'm curious as to the mechanism at work behind your "Hit Early"
recommendation. Is it your feeling that by hitting the virus early, hard and fast - that the immune system has a chance to "train" itself in a way such that it can manage HIV infection by itself once the person goes off the HAART therapy?

Dr. Kaiser replies:

Within the initial six months of infection, the HIV virus is hard at work "burrowing" itself deeply into the immune system. It is attempting to infect as many CD4 cells and macrophages as possible. By bringing its activity to an undetectable level within the first six months of infection, you halt its progress and force it to establish a more superficial infection. This superficial infection has a much better chance of being controlled by the immune system after the first one or two years of initial antiviral medication is stopped. Some of my patients have remained undetectable for several months after stopping their antiviral medication when the meds were started soon after becoming exposed to HIV in the first place. After the viral load becomes detectable in this scenario, it often remains at a low level for a very long time without causing any appreciable loss of CD4 cells. Of course, an aggressive program of natural immune system support is extremely helpful as part of your program as well.
